Referral Pathways

Generally delivered through an intensive and time-limited period necessitated by the participant's immediate and significant barriers to plan implementation.     

Best suited for participants who usually have low level of need that may be on a crisis.

Delivered through long term care however not intensive compared to Pathway A. Usually partnered with other support networks or lower level of support coordination.       

Best suited for ongoing chronic high risk participants that needs ongoing care review
